5.0 Vendor approval process 供应商核准程序

Subject to review of the completed QA questionnaire and vendor accreditations, the QAM will determine the approval route applicable for a vendor, for example:

a)      Vendor approved without audit

b)      Vendor approved by audit.

5.1  Vendor approval without audit 无需审查，即获得核准的供应商

Vendors who meet the following criteria may be considered approved without audit in accordance with this procedure;

符合以下标准的供应商，可以根据本程序规定要求，无需审查，即可获得认可

                     1) Existing qualification to ASME NCA 3800.

    1）该供应商符合现有的美国机械工程师协会（ASME）NCA3880条规定的资质审查要求。

a)      Completion and approval of the QA questionnaire.

a) 该供应商已经填写完成质量保证（QA）问卷调查表，并且该问卷调查表通过核准。

b)      Vendor holds a valid ASME Quality System Certificate (Material) or an N Certificate, the scope of which includes the supply of materials or services relevant to PSL procurement requirements.

b)  该供应商持有有效的美国机械工程师协会（ASME）质量体系认证（材料），或N认证，其认证范围包括以下材料或服务的供应：即，该材料或服务是管架集团有限公司（PSL）采购要求相关的材料或服务。

              c)   An uncontrolled copy of the vendors QA manual which confirms programme compliance with                              the requirements NCA3800 is available.

              2)   A vendor qualified and nominated by a BPS client.

Evidence to demonstrate approval i.e. ASME QSC accreditation certification or other objective documents supporting client approval, will be retained within the QA Dept.

5.2  Vendor approval by audit需要通过审查，再获核准的该供应商

If upon receipt of the completed QA questionnaire, the potential vendor has indicated that their quality programme has been accredited in accordance with another recognized QA specification i.e. BS EN ISO9001-2008 or that the vendor holds no formal accreditation, after consultation with the Procurement Manager, the QA Manager will determine if vendor approval by means of QA audit is required.

Any vendor considered not suitable will be advised accordingly.

The vendor will be notified if approval by audit is required.

The audit scope will be based upon the requirements of the PSL NQA programme and ASME NCA3800 for material organizations plus any project specific requirements identified within client specifications.

The scope of the audit will be established and agreed by the vendor prior to the audit.

Audit activities will be performed strictly in accordance with NQAP16/02 – Audit procedure.

Vendors that have demonstrated by audit that their documented QA programme and the successful implementation of any agreed corrective actions to ensure compliance with the BPS Nuclear QA programme, will be included within the nuclear approved vendors register.

5.3  Subcontracted Calibration Services 分包校正服务

As an alternative to survey and audit, vendors of subcontracted calibration services accredited by United Kingdom Accreditation Service (UKAS) through the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Arrangement (MRA), will be acceptable provided the following requirements are met:

(b) The published scope of accreditation for the calibration laboratory covers the required measurement parameters, ranges and uncertainties required by PSL.

5.4  Vendors performing sub contract Non destructive examination including ASNT-SNT-TC-A level 2 and 3 qualification.

Such vendors will be considered approved subject to providing:

a)      Evidence that Level 3 and Level 2 operatives hold appropriate ASNT qualifications and have relevant experience in NDE activities.

Copies of NDE procedures and operator qualifications are retained on file by BPS QA department.

Vendors performing Level 2 examinations will be subject to annual audit by BPS QA department.

5.5  Registered professional engineers 注册专业工程师

Registered Professional Engineer’s will be approved in accordance with NQAP 04/02 – Approval of RPE’s.

6.0  Approved vendor register 已核准供应商登记薄

The approved nuclear vendor list is controlled and issued by the NQA Engineer.

This register records the following information:

The register is subject to annual review by Procurement and QA Managers to ensure that it meets the requirements of BPS nuclear procurement strategy. The results of this review will be incorporated with the annual management review of the effectiveness of nuclear QA programme

7.0  Vendor performance  供应商绩效

Vendor performance will be monitored during routine quality activities imposed by BPS.

Such activities may include:

• Witness during manufacture at agreed intervention points as specified on the approved inspection and test plan.

• Inspection of materials upon receipt during good inward activities

• Monitoring the accuracy of documentation submitted.

• Delivery performance in accordance with the schedules defined in the BPS purchase order.

Should a vendor’s performance be considered unacceptable then this could result in suspension of a vendor’s approval status pending a further review of the vendor’s activities.

Such a review may take the form of formal correspondence or further visits to the vendor’s facility. The QA Manager will determine the appropriate course of action.

Records of any action taken will be formally recorded and retained on file for future reference.

An annual review of performance of all approved vendors will be performed by the Procurement and QA Managers to establish the vendor’s performance has been consistent to ensure that the requirements of the BPS NQA progarmme are achieved. The results of this review will be incorporated with the annual management review. Refer to NQAP02/01.

                 (a) Actual results of all required chemical analysis, mechanical tests and NDE

                 (b) A report of all weld repairs performed on the material

                 (c) Radiograph films required for material and weld repair except for those required for welding material test

3.3.3 The Receiving Inspection Report (RIR Exhibit 0702-3) shall be prepared for documenting that the receiving inspection has been performed and vendor’s documentation has been reviewed.

     应编制来料检验报告(RIR 附件0702-3) ，证明已经执行来料检验，供应商文件已经过评审。

The RIR shall be provided for spaces to record all characteristics required by the applicable procurement document and the Code, including traceability of received materials to the CMTR

来料检验报告应为空白文件，以便记录下采购文件和规范中要求的所有材料特征，包括来料对认证材料试验报告（CMTR）的可追溯性。

        The Receiving Inspection Report raised by QC Inspector, when he finished his inspection and documented on the corresponding place of RIR, the RIR with related supplied item documents such as CMTR or C of C will be sent to the NQA Engineer for his review. When all Code requirements are satisfied, the NQA Engineer shall sign and date on it and submit to QA Manager for approval.

     来料检验报告由质量控制检验员（QC）负责，该检验员完成检验工作后，将检验结果记录到来料检验报告上的相应区域，然后将此来料检验报告随附相关的供货物项证明文件，例如认证材料试验报告（CMTR），或符合性证明书（COC）一并提交给核质量保证工程师（NQA），由其审查。如果符合所有的规范要求，则核质量保证工程师将在来料报告上签名，并注明日期，然后将其提交给质量保证经理（QA），由其核准。

3.3.4 Identification 标识

        (1) If the received items are acceptable, the QC Inspector shall attach a PASSED Label to the items. For the item such as welding electrodes, and bolting materials which are small and numerous in quantity, the PASSED Label may be attached on the container.

     如果来料合格，质量控制检验员（QC）应在该物项上贴附检验合格标签。如果受检物项为尺寸较小，数量较多的材料，例如电焊条和螺栓等材料，则可以将检验合格标签贴附到集装箱上。

        (2) For the unaccepted items, the QC Inspector shall issue a Nonconforming Report and attach the Hold Label to each of the nonconforming items. However, for the small and numerous in quantity items, the Label may be attached on the container.

      检验不合格的物项，质量控制检验员（QC）应发出不合格报告，并应在每一不合格物项上贴附停用标签。尺寸较小，数量较多的物项，则可以将标签贴附到集装箱上。

3.3.5 Sampling 取样

        Receiving Inspection will be performed on a sample basis in accordance with the following:

        Quantity received ＜ 100 off min 10% per item received.

      来料数量＜100的，从每批来料物项中抽出10%检验。

                                                                                    ≥ 100 off min 10 off per item received.

      来料数量≥100的，从每批来料物项中抽出10个检验。

        Should any item be found to be non compliant a further sample based on the above “sample size” shall be inspected. Further recorded non compliance’s will result in the entire batch being rejected.

3.4   Acceptance of Services Only 服务的验收

        Services shall be accepted by any one or combination of the following methods:
